Add Location to Report 485

I noticed on my most recent Report 485 I ran on the Afton project that there is no information in the “Location” column or the “Notes” column. That would be useful.

Also, a column for anything entered in the Location/Ref# box would also be very helpful to see the more refined location notes that we often add. Any way this could be modified to show that info?
